Office Financial Policy
We feel that all patients deserve the best dental care possible… Moreover,
we feel that everyone benefits when office policy and financial arrangements
are understood. Financial concerns should not stand in the way of obtaining
the best possible treatment. In order that we may have a definite understanding
in regard to payment for dental services, the following is our office
policy:
If you have insurance…
It is important to understand
that not all services are covered benefits under all contracts. The fee
obligation, regardless of insurance coverage, belongs to the patient.
After we have determined the deductible and co-payment benefits, we will
expect your balance at time of service. Upon request. A predetermination
of benefits will be filed on your behalf.
New patients are required to
bring all insurance information with them the day of their first appointment.
If the insurance information is not given at that time, we will have
to assume that you do not have insurance and payment for the day’s
services will then be due.
If you do not have insurance…
We require
all services to be paid in full on the date of the service.

Cancellation Policy
We value our patient’s
time and try to accommodate our patient’s scheduling needs to the
best of our ability. In return, we request the same courtesy.
We require
a minimum of 24 hours notification if you are either canceling or changing
a scheduled appointment. |
